Submit Customer Service Request

If you are a person with a disability or a person representing someone with a disability who wants to submit a grievance under the Americans with Disability Act, please click here to submit your grievance: ADA Grievance Form.

For all other service requests, you can use the form below to submit a request for customer service. Requests are limited to the drop down list. If you do not find what you want to report on the drop down list then submit your request using the Traffic or Work Zone Concern form.

Most of the fields on the form are required fields. Please fill out the information completely and submit the service request. The location will be reported to the nearest maintenance facility for evaluation.

Customer Service Requests are handled Monday through Friday, 8AM to 4PM. This form should NOT be used to report any kind of highway emergency. Report highway emergencies to 911 or the California Highway Patrol immediately.
